Miscellaneous Services Form and affidavit can be downloaded from the website http://www.hcizambia.gov.in . Forms are also available at the High Commission.
Documents required:
Miscellaneous services form duly filled
Passport of both spouses ( original and photocopy - first two pages, last page and first entry into Zambia)
Two recent passport size photographs (see ‘Photograph guidelines’)
NRC/Residency Permit (original and photocopy)
Documentary evidence of marriage such as wedding card or photographs and photocopy
Affidavit confirming the marriage duly notarised by a Notary Public or certified by a gazetted officer in India. The date and place of marriage should be mentioned in the Affidavit. (original and photocopy)
